Abstract: 

ABDOMINAL mass is one of the commonest clinical findings in children. The purpose of this study was to evaluate ABDOMINAL MASSES in different age groups in a referral children surgical center. We retrospectively reviewed records of 325 patients (57% boys, mean age 37.7 monts) admitted to Amir Kabir hospital in a period of 12 years for ABDOMINAL mass surgery. The 4 most common ABDOMINAL MASSES was Wilm's tumor(22.4%), ABDOMINAL lymphoma(13.5%), neuroblastoma (12.5%) and hydronephrosis (12.3% ).These MASSES composed 60% of ABDOMINAL MASSES and the remaining were: hydatid cyst of liver, ovarian MASSES, liver MASSES, choledochal cyst. mesentary and omental cyst, etc.This results were found to be similar with little differents to results of other centers.

Abstract: 

Background/Objective: Detection of the tissue diagnosis of ovarian space occupying lesions (OSOL) has remained a challenging task for sonographers since many adnexal MASSES have nonspecific sonographic appearances. Our objective was to evaluate the accuracy of the ABDOMINAL sonographic diagnosis of adnexal MASSES in 79 women with a known OSOL undergoing laparotomy for ovarian MASSES in Tabriz Alzahra's Haspital, northwestern Iran.Patients and Methods: From March 2004 to February 2005, sonographic reports of each patient were compared with postoperative findings. Results: Comparison of the preoperative sonographic and final pathologic diagnoses revealed a correct sonographic diagnosis in 77% of patients. The identification of ovarian cystic teratoma was correct in 17/24 cases (sensitivity of 71% and specificity of 98%). The identification of ovarian malignancy was correct in 7/10 patients (sensitivity of 70% and specificity of 98.5%). Sonograms were frankly misread in 14/79 cases, and were missed in 4/79 cases.Conclusion: In conclusion, our results show high resolution ABDOMINAL ultrasonography is an effective method in diagnosis of ovarian tumors and on 70% of patients can differentiate malignant tumors from benign tumors.

Abstract: 

Background: Renal cell carcinoma is the third most common urological cancer. Surgical resection is still the mainstay of treatment for this tumor. Here we present a new surgical approach for the management of locally advanced renal cell carcinoma.Methods: We chose ten patients with extensive renal MASSES. The patient, under general anesthesia after preparation and draping, was placed in the supine position. We made either a right or left classic subcostal incision which was then extended as a midline incision through the linea alba to the lower abdomen. After mobilization of the right ascending or left descending colon, the renal artery and vein were detected, ligated and divided. Next, the involved kidney was released from the adjacent structures and removed, including Gerota's fascia.Results: The study included 6 men (60%) and 4 women (40%) with a mean age of 57 years. Mean blood loss was 1500 mL and mean operative time was 180 minutes. The patients' surgical wounds were checked for two weeks after the operation, none of the patients developed wound infections or dehiscence. At the time of writing this manuscript, in January 2011, 7 patients (70%) were alive with no evidence of ABDOMINAL herniation at the surgical site.Conclusion: A subcosto-midline incision or anterior triangular flap incision is a modified ABDOMINAL incision. In our experience, this incision is useful for the resection of locally advanced large renal MASSES.

Abstract: 

The aim of this study was to evaluate the role of fine needle aspiration cytology in the diagnosis of ABDOMINAL and retroperitoneal MASSES in children on this study. In 53 cases of childhood ABDOMINAL and retroperitoneal MASSES within a 4 year period (1998- 2001) preoperative fine needle aspiration was done under the guide of CT scan. 2 pathologists reviewed fine needle aspiration smears. In all of the cases the gold standard for diagnosis was the tissue specimen, which showed 37 malignant, 2 benign neoplastic and 12 nonneoplastic diseases.This study showed that the sensitivity and specificity of fine needle aspiration cytology for the diagnosis of malignancy and benignity (positive or negative for malignancy) is 97.2% and 81.2% respectively. It was 100% accurate for the diagnosis of cell type in neuroblastoma-ganglioneuroblastoma, hepatoblastoma and Wilm"s tumor, 77% accurate for lymphoma and 57% for germ cen tumors. There was no complication in any ofthe cases after fme needle aspiration. So fine needle aspiration is a reliable and sensitive method for the preoperative diagnosis of malignant pediatric ABDOMINAL and retroperitoneal MASSES and we recommend doing FNA cytology as a routine method for the diagnosis of such cases.

Abstract: 

ABDOMINAL MASSES in neonatal period is one of the main causes of patients bed ridden in infantile surgery wards. The rapid encounterance with these patients has caused a decrease in mortality and morbidity.This study is a kind of historical research which has been conducted on data base management systems of two above mentioned hospitals between years 1361-1378. The final outcome was only 25 cases. From these neonates, 68 percent were male and 32 percent were female. Most of the patients were in their first neonatal week. The etiologies related to urinary system (56 percent), were found to be in the first place. Tumors of different origins (20 percent), ovarian MASSES (12 percent) and gastrointestinal tract (12 percent) had their own places respectively.Majority of cases have had surgical operations (92 percents). The case fatality rate among these patients was 24 percent which most of it was because of benign causes. The rate of mortality in male was much larger than female neonates.We concluded that the inspection of embryo’s urinary system with ultrasound and primary examination of neonates, is vital and in most of the cases, an urgent surgery is indicated. Also laboratory assessment and radiological examination in suitable therapeutic procedures in first 48-72 hours is recommended.

Abstract: 

Background and Aim: Chronic low back pain is one of the most common clinical problems for which various treatments have been carried out so far. Considering the ABDOMINAL drawing maneuver effects on the performance of lumbopelvic muscles and the effect of these muscles themselves on the lumbar lordosis, and also having in mind the relationship between lumbar lordosis and low back pain, the present study was carried out to investigate the effect of ADIM on the lumbar lordosis angle in patients with chronic low back pain. Materials and Methods: A total of 15 women with chronic low back pain and lumbar hyperlordotic posture took part in the study. To measure the lumbar lordosis angle in standing posture, we used the Cobb method in radiographies with lateral view. Results: The mean difference between the lodosis angles (before and after applying the maneuver) was 11. 276, meaning that prior to applying the ADIM, the lordosis angle was more than that after applying it (P<0. 05). Conclusion: According to the findings, applying ADIM in standing position in patients with chronic low back pain and lumbar hyperlordotic posture can result in lordosis decrease and lumbar alignment improvement and it also decreases the chronic pain in this region.
